feat(context): tambah context::shaping (port dari shortsend)

Perilaku should_shape/shape_messages tidak berubah, hanya sumber
penghitungan token yang sekarang lewat context::tokens (tiktoken-rs)
menggantikan heuristik char/3 bawaannya sendiri.
